Out of Hours
If you need urgent medical attention when the surgery is closed you can ring direct on 111. Otherwise Blackhall & Peterlee patients can call 0191 586 4331. Your call will be automatically transferred to an message, which will explain the options available to you. Jupiter House patients call 0191 587 2488, this will divert you direct to 111.

 

 

Appointments
These are made via the Reception Desk at all sites or via the phone. You may request to see a particular Doctor but this may involve a longer wait for the next available appointment.

Home Visits
These are for patients who are housebound and too ill to come to the surgery. If at all possible, please make requests for these before 10.00 a.m. This allows the Health Care Professional to plan their rounds

NEWLY REGISTERED PATIENTS

 It may take some time to receive your medical records from your previous Doctor. In order that we can obtain some background information from you, a brief ‘New Patient medical’ will be carried out and you will be asked to complete a questionnaire about your health and bring a urine sample with you. This is normally carried out by the Health Care Assistant.

It is very important to provide us with information about you and your family, so please take the time and trouble to attend. If you are on repeat medications you will also be asked to make an appointment with a doctor for a medication review prior to these being placed on repeat. If you miss your first registration appointment you will NOT be allowed to register with the practice. Please discuss booking an appointment with our receptionists if you wish to join the practice.


Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
